Case Management
The Safe Entry Community core service is case management. The focus is to serve those individuals with the greatest needs who are consuming the most costly, intensive, and intrusive resources. SECMHC’s Case Management Service is based on the Assertive Community Treatment approach. The consumer is the center of a circle of supports. Those supports consist of family members or significant others the consumer wants to have involved in their life along with an SECMHC multidisciplinary team.
SECMHC has developed specialty teams within case management to better assist individuals with special needs. These teams include:
FACT -Forensic Assertive Community Treatment
PATH -Program for Assistance in Transition from Homelessness
MOT -Mandatory Outpatient Treatment
How does SECMHC do this within the confines of a treatment facility? They don’t. The majority of case management services are delivered in natural settings, such as the home, or school, or place of employment.
The key to success with the consumer and his/her team is their relationship. This team includes a psychiatrist or nurse practitioner, registered nurse, case manager, and case manager supervisor. Working together, the team helps the consumer see how the illness impacts every area of his/her life. The focus is on reducing the impact of the illness and rebuilding his/her life. |
